The role of bedside ultrasonography for occult scaphoid fractures in the emergency department.

چکیده
BACKGROUND Our aim is to study the accuracy of emergency medicine (EM) physician-performed, bedside ultrasonography (BUS) in patients with clinical suspicion of scaphoid fracture and normal radiographs. METHODS From January to December 2011, an EM physician used BUS to prospectively evaluate patients presenting to the emergency department (ED) with clinical suspicion of scaphoid fracture and normal radiographs. BUS examination of the scaphoid was conducted prior to a wrist magnetic resonance imaging (MRI) scan, within 24 hours of wrist trauma. The outcome was determined by official radiology reports of the MRI. The results were compared using the chi-square test. RESULTS Of the 63 enrolled patients, 12 Patients were BUS-positive. Of these, MRI results agreed with the BUS findings in 12 patients who had cortical damage of the scaphoid with hematoma. In 35 instances, hematoma with no cortical damage was detected with BUS and corroborated by MRI. A scaphoid fracture was demonstrated by MRI in two patients from this group. The sensitivity, specificity, positive predictive value, negative predictive value, and negative likelihood ratio for BUS were 85.7%, 100%, 100%, 100% and 0.14, respectively. Accuracy of BUS was not statistically different from MRI. CONCLUSION The diagnosis of scaphoid fractures is another BUS application in the ED. EM physicians should consider diagnosis of scaphoid fractures using BUS in the emergency department.

Bedside Ultrasonography for Early Diagnosis of Occult Radial Head Fractures in Emergency Room: A CTComparative Diagnostic Study
Background: Some of the Mason type I fractures cannot be detected on early radiographic images. These occultfractures are considered as a diagnostic challenge for physicians. Our aim was to determine the value of bedsideultrasonography for the detection of Mason I radial head fractures that are non-visible in early X-ray’s.Methods: A prospective blind single-center diagnostic study was conducte...
متن کاملInter-observer agreement between 2-dimensional CT versus 3-dimensional I-Space model in the Diagnosis of Occult Scaphoid Fractures
Background: The I-Space is a radiological imaging system in which Computed Tomography (CT)-scans can be evaluated as a three dimensional hologram. The aim of this study is to analyze the value of virtual reality (I-Space) in diagnosing acute occult scaphoid fractures. Methods: A convenient cohort of 24 patients with a CT-scan from prior studies, without a scaphoid fracture on radiograph, ye...
متن کاملCost and clinical effectiveness of MRI in occult scaphoid fractures: a randomised controlled trial.
BACKGROUND Clinical and radiographic diagnoses of scaphoid fractures are often challenging at the time of injury. Patients are therefore usually reassessed which has cost implications. Various investigations exist but MRI has been suggested as effective in diagnosing these injuries early. AIM To determine whether early MRI in suspected occult scaphoid fractures is more clinically and cost eff...

متن کاملIntroducing a Clinical Practice Guideline Using Early CT in the Diagnosis of Scaphoid and Other Fractures
OBJECTIVE We developed and implemented clinical practice guideline (CPG) using computerized tomography (CT) as the initial imaging method in the emergency department management of scaphoid fractures. We hypothesized that this CPG would decrease unnecessary immobilization and lead to earlier return to work. METHODS This observational study evaluated implementation of our CPG, which incorporate...
